How to Get Domicile Certificate:

Instructions For Issuance of Domicile Certificate in District Lakki Marwat

In order to obtain a domicile certificate, an individual / Applicant should follow the following Instructions:

1. Domicile Certificate must be verified & attested by:

• Kanungo
• Patwari
• Gazetted Officer/Union Council Secretary /General Counselor.
• Domicile Certificate must be verified & attested by:
• Naib Tehsildar
• Deputy Commissioner/Assistant Commissioner

2. Two (2) Passport size photographs front side attested from Gazetted Officer / Union Council Secretary /General Counselor.

3. Original & Copy of Applicant CNIC or Form ‘B’ for under 18 years of age.

4. Original & Copy of Parent’s/ Guardian’s CNIC.

5. Applicant must be available personally at the submission time of Domicile Certificate.

6. Mobile number must be written on the back side of Domicile Certificate.
